Job Description:\r
\r
\r
\tAssists the physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs, as well as in developing the resulting plan of care/treatment including appropriate long-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.\r
\tCompletes initial and ongoing comprehensive assessments at appropriate time points as assigned.\r
\tParticipates in ongoing interdisciplinary assessment of the patient.\r
\tParticipates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).\r
\tProvides services that are ordered by the physician as indicated in the plan of care.\r
\tProvides interventions and therapeutic treatment in accordance with the physician certified plan of care and occupational therapy professional standards of practice.\r
\tSelects and teaches task-oriented therapeutic activities designed to restore physical function, and sensory-integrative function.\r
\tTeaches compensatory techniques to improve the level of independence in the activities of daily living (ADLs).\r
\tDesigns, fabricates and fits self-help devices.\r
\tRegularly assess changes in the patient’s status since the last visit/treatment day, and determines whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the physician as indicated.\r
\tAdvises and consults with the family and/or other caregivers to promote patient progress toward mutually agreed upon goals and planning for discharge.\r
\tCounsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.\r
\tEffectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge.\r
\tCommunicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.\r
\tCompletes and submits timely, legible, accurate, relevant and sufficient clinical documentation (written or electronic) addressing the treatment and interventions provided, the patient’s response to the treatment and interventions, and the patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and payor requirements.\r
\tParticipates in Quality Assessment and Performance Improvement (QAPI) program.\r
\tParticipates in in-services or training, as requested.\r
\tParticipates in the Office’s Emergency Preparedness program, as requested.\r
\tSupervises professional and paraprofessional staff, including occupational therapy assistants, as assigned.\r
\tCompletes other assignments as requested and assigned.\r
\tAccesses personal health information (“PHI”) necessary to fulfill the above duties and responsibilities.  Access to use and ability to disclose PHI is further defined by each organization/department.\r

Job Requirements
Minimum Education & Experience Requirements:\r
\r
\r
\tCurrent licensure or certification, if applicable, as an occupational therapist in the state(s) in which he/she will practice.\r
\tA graduate of an occupational therapy education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA), or its successor organizations of ACOTE and is eligible to take, or has successfully completed the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).\r
\tIn the last five (5) years, two (2) years of experience as an occupational therapist that is relevant to the patient population assigned preferred.\r

\r
Knowledge, Skills & Abilities Required:\r
\r
\r
\tAble to provide proof of current CPR certification, per organization-specific policy.\r
\tAble to demonstrate basic understanding of computers and workplace software.\r
\tAble to work with people, function well under stress, use clinical judgment, be creative and accept challenges.\r
\tAble to effectively communicate in English with all levels of the workforce, both clinical and non-clinical.\r
\tAble to perform and prioritize multiple functions or tasks.\r
\tAble to read and interpret technical instructions related to the care of the patient and use of equipment in providing treatment.\r
\tAble to visually and aurally observe and assess the patient.\r
\tAble to effectively deal with multiple changes based on patient needs and scheduling.\r
\tAble to provide proof of valid driver's license, if applicable.\r
\tAble to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport patient.\r
\r
\r
Working Conditions & Physical Effort:  \r
\r
\r
\tAble to travel constantly within geographic area serviced by office from assignment to assignment.\r
\tMeets the applicable health requirements to provide patient care per applicable law or regulation.\r
\tWork is normally performed in a residence, aggregate living facility, school or occupational setting.\r
\tOccasional exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment (e.g. face masks, goggles, latex and or non-latex gloves).\r
\tAble to work flexible schedule and/or evening hours, as needed.\r
\tAble to constantly stand, walk, bend, stoop, squat, kneel and reach freely.\r
\tAble to constantly grasp with thumb in opposition to fingers of palm, i.e. manipulate pen, knobs or objects.\r
