GENERAL SUMMARY: Under general supervision of the therapy supervisor, implements treatment programs and organizes and conducts a variety of sensorimotor, educational, recreational and social activities designed to help clients regain physical or mental functioning and\/or adjust to their handicaps. PRINCIPLE DUTIES AND RESPONSIBILITIES : Provides skilled occupational therapy services to clients in their homes. Provides initial and ongoing comprehensive assessments, including the Outcome and Assessment Information Set (OASIS) assessments, as required. Provides an assessment of living environments and recommends adaptations to living and working environments, including use of equipment. Evaluates and re-evaluates, as required, client\u2019s level of function by administering diagnostic and prognostic tests and discharges services when appropriate. Develops and implements treatment programs in accordance with the objectives of the physician\u2019s plan of treatment and goals after evaluating the client. Promotes and teaches client independence in essential life skills related to self-care mobility, social adjustments and\/or vocational pursuits. Maintains clinical records and professional documentation by preparing evaluation and progress notes, discharge summaries and related documentation. Works in conjunction with Home Health Care staff in the planning of occupational therapy treatment programs. Participates in multidisciplinary case conferences to enhance the development, progression and evaluation of the client\u2019s plan of treatment. Participates in professional organizations and inservice educational programs. May supervise the activities of occupational therapy assistants, home health aides and students. May conduct or participate in inservice education programs in occupational therapy for Home Health Care staff and other System employees. EDUCATION AND EXPERIENCE: Four (4) year baccalaureate degree program accredited by the American Medical Association and the American Occupational Therapy Association. Registration by the State of Michigan as an occupational therapist. One (1) year of clinical experience.
