Occupational Therapist , OT
This is a full time, benefited position working in Gardner surrounding areas.
Plans and administers occupational therapy services to patients in their place of residence and in community programs according to physician’s orders. Administers to patients various evaluations and therapeutic procedures while maintaining agency records. Patient population may range from birth to over 100 years of age with impairments not limited to: muscular skeletal, neurological, orthopedic resulting in functional limitations and or disabilities due to injury, illness, disease or other causes. Adhere to all agency policies including, but not limited to, confidentiality agreement, HIPAA, personnel policies, clinical policies and procedures, professional code of ethics and State Practice Act.

Successful completion of an occupational therapy program, at a school of Occupational Therapy approved by the American Occupational Therapy Association. If licensed or registered prior to 1966, meets the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12 or (1) (2) (3) (4).
Licensed by the Commonwealth of Massachusetts Board of Allied Health, to practice Occupational Therapy.
Recommend a minimum of one to three years’ experience in Occupational Therapy, preferably in a home health setting or rehabilitation facility.
Demonstrate knowledge and skills necessary to provide care to and communicate with primarily the adult and geriatric population and to a lesser degree the pediatric.
Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
Demonstrate ability to exercise independent judgment.
Lifting and moving clients and equipment that may exceed 30 pounds is required.
Job requires one’s own transportation. Must have reliable, safe transportation and automobile insurance coverage. Valid and current driver’s license.
Demonstrate ability to adapt to patient status and needs. May be frequent schedule changes due to staffing, patient condition and new referrals.
Demonstrate verbal and written communication skills, interpersonal skills, organizational skills, and presents self in a professional manner.
Basic knowledge of computer use, adequate typing skills.
Demonstrate importance and adhere to agency policies regarding password-protected data.
